Default Clutch install Fail

i did a clutch yesterday and after 100 yards out of the shop it popped out of gear and now the clutch is not engaging. You can shift through all the gears with the engine running and nothing, i will be tearing it apart this morning to see what the root cause is.

i cant think of anything i did wrong during the install.

my worst case right now is the trans went bad.

any thoughts?
