It's that I haven't been able to pinpoint the problem. As you can see from this thread it is different for everyone, which doesn't help in locating the problem.

Now I won't lie, this isn't a very high priority issue for me. So when I have time I look into it some more, but I'm not going to spend every waking minute on this.
